Application procedure:
Application Forms and Information Brochure will be available at DMAT office E-2/51, Arera Colony, Bhopal, Modern Dental College & Research Centre, Indore, People’s College of Dental Sciences & R.C, Bhanpur, Bhopal and R.D.Gardi Medical College, Ujjain from 19.02.2009 on payment of Rs. 1,500/- by cash or D.D. in favour of APDMC payable at Bhopal. Application Forms can also be obtained from above Centres by post by remitting D.D of Rs.1,600/- (including postal charges). Filled Application Forms should reach to the DMAT office Bhopal latest by 14.03.2009 along with Demand Draft of Rs.1,000/- as Examination fee in favour of APDMC payable at Bhopal. Application Form can also be filled online from the website and can be submitted along with a Demand Draft of Rs.2,500/-.
