Application Form correctly filled in ONLINE on prescribed format and printed copy alongwith Demand Draft of Rs. 500/- drawn in Favour of Director, SGPGIMS (Academic Account) payable at State bank of India, SGPGI Lucknow and supporting documents / certificates should be submitted to the Executive Director, SGPGIMS, Lucknow by Registered Post / Speed Post / Courier.
Last Date for submission of application form is 20 June 2014
Candidates are required to mention their email address in the form since information for entrance test will be intimated through email.
